I understand that Health Assessments (including vaccination status) must be completed/updated for all students age 5 and under, as well as all NEWLY enrolled students of any age.

 

I understand that The State of Connecticut Dept. of Public Heath requires all students between 24 to 59 months (4.9 yrs) of age to receive the influenza vaccination, and that I will submit documentation from our primary care doctor when my child receives his/her vaccine.
